How could an error on the DS-260 form impact my case?

I recently submitted my DS-260 with the assistance of an agent. After submission, I realized that the agent put “No” to the question: Have you ever been refused a USA visa. I was refused a visitor visa about 20 years. I told the agent about this but she still put the wrong answer there. Will that be a problem for me at the interview?

It is an issue that needs to be raised, either by email now to the consulate, so you create a record (i.e. a timely retraction) or definitely at the start of your immigrant visa interview. As long as the visa refusal reason was not for any serious inadmissibility ground, it is unlikely to affect the outcome of your immigrant visa interview. Just make sure you are completely honest and upfront in all of your answers and explain that you did not intend to "omit" that information or answer incorrectly. Bring it up before the interview begins.

You may be able to amend now, but if not, you can correct errors at the beginning of the interview, but be prepared to explain that with a lot of detail, even resulting in a rescheduling of your interview or a denial since they do not do requests for evidence.
